RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT
Participation in physical activity and conditioning programs carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. I knowingly and freely assume all such risks, (both known and unknown, even if arising from the negligence of others) and assume full responsibility for my participation. I certify that I am more than 18 years of age, and that this waiver remains in full force and in effect until specifically revoked in writing by me.
