Request for Transcript Re-Evaluation Form
1. After completing a "Change
of Academic Intent" to declare a new major, this form should be submitted to
request a re-evaluation of your transcripts, if you have additional coursework
that may apply to your new major.
2. Students who have not declared a major will
not have an evaluation completed. Transcript evaluations are only completed for
students in a declared program of study.
3. If you have not taken courses in more than
two years, you will need to re-submit all your transcripts before a
re-evaluation can be completed.
4. Requests for re-evaluation take up to three
weeks to be processed.
